This is the link to the FMS Bullying Report Form. Once the form is submitted, an email is sent to the school counselor, principal, and assistant principal to begin investigation of the situation being reported. |
When Should You Fill Out a Bully Report Form? If you can check ALL 4 boxes, definitely fill out a Bully Report Form. If you check less than 4 boxes, fill out a Request to See the Counselor Form, so that we can talk about the situation. If it is considered bullying after we talk, we will fill out the Bully Report Form together in the Counselor's Office. |

About: The Missouri School Violence Hotline (SVH) began in October of 2001. In 2019, through the Missouri School Safety Initiative, the reporting mechanism moved to the Missouri Information Analysis Center for 24/7 operation and was named Courage2ReportMO. The goal is to make schools safer by helping school districts and law enforcement learn about school violence as soon as possible. Courage2ReportMO takes ANONYMOUS reports involving any public or private school in Missouri with students in pre-kindergarten through 12th grade.
What should be reported? Any violence that happens on school property or the school bus should be reported to C2R. This may include:
Officials with the school and law enforcement will then determine how to appropriately handle each report.
